Issues in descriptions of the AuthZ Server Metadata attributes (ITI-103 response):

issuer: wrong example.
scopes_supported: cannot be optional, is required in IHE IUA.
response_types_supported: cannot be optional, is required in IHE IUA.
token_endpoint_auth_methods: in IHE IUA, this attribute is named token_endpoint_auth_methods_supported.

Kept optionality for scopes_supported, since it is optional in IUA and SMART on FHIR. Fixed the other issues.
