update the sequence diagrams #98

only ehr launch is supported -> need to update the sequence diagrams

In the same diagrams: /metadata should be /.well-known/smart-configuration

4.5.1 mHealth App is a "Patient Portal" introduce a new 4.5.2 (copy of 4.5.1) with the following modificaitons Patient Portal -> Healthcare Professional Portal or Primary System Patient -> Healthcare Professional 4.5.2 original -> new 4.5.3 patient get document with smart-on-fhir option (EHR Launch) (4.5.3) mHealth App is a "Patient Portal" mHealth embedded App -> "SMART on FHIR App" 4.5.3 original -> 4.5.4 hpc get document with smart-on-fhir option (EHR Launch) (4.5.3) mHealth App is a "Healthcare Professional Portal or Primary System" mHealth embedded App -> "SMART on FHIR App"

SMART on FHIR App -> needs also do the GET /metadata

in the smart on fhir ehr launch we need to replace steps [02]-[14] (en of ITI-83 request) with the one from the generic option. (only one smart on fhir launch in the whole sequence).
