Pages are being redirected to /index.php #188

I think I mentioned that EZP said they couldn't find any issue, but we're also seeing a lot of .php errors (will send screen shots via email) and I still am getting redirected to /index.php once in a while, particularly when using chrome.

Could you please have a look at the .php errors when you get a chance?

Nov 10/17 Installed a patch that should fix the cause of warnings from ctools module. Also did recommended module updates (minus the Media module).
