Change review form to transition into new user form with JS

Using JS to replace the review form with the signup form when creating a new user, removing the need for a separate signup page.

Both forms will be displayed on index.php

• interrupt form submission w/JavaScript
• AJAX call to API to see if the user exists
• replace review form in div to user sign-up form
