Changes:

Refactor playlist filter function to lower komplexity
Fix errors in some db migrations on down side
Use collation latin1_bin for ids, enums and sort order
Improve indexes
Remove orphan rows with relations to deleted courses and set foreign keys to prevent this in the future

Solve #927
