Also added a light shadow missing from document-save. 16px action icons generally don't seem to keep the shadow from larger sizes, but this document square looks fairly flat without it (the border gradient seems to depend on it a bit). Hopefully that's okay (if not maybe adjusting the border gradient for the 16px document square would be another solution).
