Could this be a duplicate of: https://github.com/elementary/gala/issues/115 ? It would be interesting if you could also include whether you use one of more monitors and whether those are HiDPI / LoDPI.
Could be similar indeed. On closer inspection I noticed this is also only happening when hovering the elementary top bar, desktop, dock and windows. Once the cursor enters an app, it gets resized to the required size.
I'm using a Dell XPS 15 9560 with the internal monitor disabled (using the super + p cycle), but it also happens if the built-in monitor is enabled.
I had to restart a lot to test things and more than often I got a cursor at the wrong size. In LoDPI a double sized cursor and the other way around. I could not read a pattern after restarting many times. So getting the cursor at the right size was just a matter of luck.
Also, the tiny cursor is application specific. In Firefox for example is the cursor small on the window title bar:
