Review index sizes #743

Over-constrained hosts as in #742 may have problems with other indices, like the slug index on wp_wp2static_addons.

varchar(191) is used in the post_name index on wp_posts, so we should be able to use that everywhere.

I'll address in https://github.com/leonstafford/wp2static/issues/718 for the addons. Can close this one when core is deemed good
