Set DOCTYPE to be PubMed version 2.7 #69

To be explicit about which version of PubMed to support, set it to version 2.7 in the DOCTYPE of the output file. Currently we are using 2.6

Changes since 2.6 in the PubMed dtd,


Changes:

  | - added optional @EqualContrib to Author
  | -added <CoiStatement> in <Article> for Conflict of Interest statements
  | - changed all entity declarations for character files to use .ent filenames
