Upgrade FOCA to version 0.13.0

https://github.com/PacificAnalytics/pa-DRS-Crypt4GH-PoC is a fork that uses foca 0.12; I assume that a further upgrade to 0.13 wouldn't be a big lift. If nobody is working on this I can port over the changes.
