Add section on upgrading to ELm 0.19 #27

This is the change that I noticed when upgrading a project, but there are probably more. Hopefully this will help others, and having an upgrade section will hopefully attract further upgrade notes.

Cool! I like the idea of having upgrade instructions. I try to diligently note everything in the CHANGELOG.md, but hadn't really considered putting upgrade instructions anywhere.
