Review and cleanup Annotation support #200

Areas of concern:
  - Not all stax2 implementations support Attribute2/Namespace2
  - EdmxFormatParser.getAnnotations relies on this, catches Exception!
  - EdmxFormatParser#isExtensionNamespace is defined using schemas.microsoft.com
  - MetadataProducer

Too big for 0.7, punting for now.
