Added a ladder that works with apple tv and browsers

This is the ladder that has been working great for apple tv. I tested elv config iq_xxx with both a 720 and 4k stream to ensure it is being applied.

I basically removed the 360 rung and replaced it with 540 low quality rung.

The live stream app will need to pull these changes or something similar @elv-zenia
