doctor lost after diabling 电话咨询和预约 + 文字咨询开通、关闭功能 #89

1. 如果一医生的电话咨询和预约都关闭的话， 
专家列表就看不到该医生的信息了。这是不对的。电话咨询��
�预约的开通和关闭应该只影响按钮的显示。

2. 文字咨询开通、关闭功能:
医生管理员：登录进入医生管理-》编辑某医生-》编辑页面添
加文字咨询开通、关闭设置（文字咨询设置：开通、关闭 
ratio button）
医生本人：登录进入编辑档案->编辑页面添加文字咨询开通、
关闭设置（文字咨询设置：开通、关闭 ratio button）

医生信息表增添文字咨询开通信息.

Andy在此基础上，实现医生文字咨询开通、关闭特性。

Done the following.

关闭设置（文字咨询设置：开通、关闭 ratio button）

是否开通文字咨询可查询DoctorCap表的acceptquestions字段。
