Okay, so here is the cosmetic changes only.

I added phpcs to composer in require-dev (composer install --dev), which is an awesome little util to help linting php code. I added a Makefile rule to run it with "make lint". With this patch set applied, it finds no issues.

This has be merged into PU branch. Very light changes as follows...

phpcs XML was moved to top-level (shouldn't be hiding in test directory).
Removed reporting from phpunit.xml.dist, as the distributed example should be bare bone. Reporting differs between people & organizations.
Ran xmllint on both xml files.
