Polishing the manuscript #123

This is quick pass on the content of the manuscript. It looks ready to submit to me. @emhart is there anything else you need help with? Could we submit it soon?

@emhart I think you should merge this, then I will go through and pull out html comments, then submit? Or, do we leave them in because they won't actually be submitted (since we are just submitting the pdf for review)? Either way, I can take one more quick pass through today after you merge this PR.
