DEAN role activation #202

DEAN role to replace TOA role.

Will need to assign each DEAN to a respective academy within the IU.
Each DEAN role will have permissions to add and edit course notes and their exams within their respective academies.
Assigning courses to a specific academy will add it to their individual access permissions.
The ability to activate and deactivate courses.
Notifications for TO (CC: CA:TO) for when courses are update/added with action officer.

Three DEAN roles will be needed for now, one for each: IWATS, CSMA, & SA.
