Admin: Generate a Report form has no instructions

JAWS Only testing:

1. Log on to the application and move to the Generate a Report page at the following URL: https://forest-service-trees-staging.app.cloud.gov/admin/christmas-trees/reportsp
2. Review the form and note no requirement instructions are provided.
3. Press Spacebar on the "Permit Number" radio button to check it.
4. Press the Tab key to move to the "To generate a report, please enter a permit number" edit field and type in a 6 digit number.
5. Activate the "Get Report" button. No discernable action is taken. No error message is spoken.
6. Begin reviewing the form. Find an error message stating "permit number requires a 8 digit number." Instructions must be included noting form requirements. If user had been aware that an 8 digit number is required, user could ensure the requirement is met before activating the Get Report button.

Instructions must be provided at the beginning of a form allowing the user to understand what is required to successfully submit the form before completing form fields.

I think I noticed a small typo when @sadlerw was demo'ing during planning meeting - should read "an 8 digit number" rather than "a 8 digit number"
