That information looks good -including basic telephone number, physical address and office hours and link to the forest website is great. I don't think it's necessary to include the fax number.
I have some updated material right here - where I took out the fax number and added in whatever information I could find regarding permit sales. I made an assumption - that no permit sale informations means you can buy them at the office during the hours listed on the website. That may not be 100 percent correct, but I figured it'll get us through the sprint. :)

Your assumption is safe in that you should be able to buy them at the office during the hours listed on the website. The attached looked great. Looks good! Just a note that AP style removes the :00 (as its considered essentially clutter that takes up valuable space). So 8:00 is supposed to be 8. :)

Would you like to add the email address for ordering permits online? The story didn’t call for it and the forest websites had mixed info. I don’t think we necessarily need an email since hopefully users will order permits online via this system. Perhaps we can test in usability if this info would be helpful?
I agree that design work would be helpful in presenting this information on the webpage. I can send a few examples through later if that would be helpful.

Remove the telephone, envelope, and clock icons for each District. And for the Mt. Hood NF, the full names of each district aren't listed, for example, "Clackamas" should read "Clackamas River", and " Hood" should read "Hood River", Zigzag and Barlow are correct. The issue of District naming should be corrected elsewhere throughout the page. The ARNF doesn't have the hours of operation for the Ranger Districts.
+1 to Amber's comments. A potential concern is that contact info is duplicated from other areas on the FS website, and could easily become out of date. Consider whether contact info should be duplicated or linked.
