Island Mode Team Support #72

Looks like BentoBox has added a team functionality. Add support for teleporting members to their team island.

Team support is already supported, looks like it was actually fallback logic that was request. This is added in 004b487c2d2565ae56556ccdae2b6002d9b0e9ba
