Strange phantom opportunity box - pressed 'Got It' once but it was still available (see pic)

At Southern Maine Community College, I clicked sign up for basic employment workshop and took it, but got no reward. Came back and the opportunity was still clickable. Clicking on it gave me this visual:

Both buttons ('no thanks' and 'got it') apparently were still there and clickable - when I clicked Got It again it started functioning normally (says it's been used, no longer clickable)

~ Went to ILAP, didn’t have the problem! Volunteer @ YMCA

** Didn’t receive a community reward "discover locations to get a job" at Catholic Charities Then got +1 C

Sign up for employment counseling @ GPCC Get a buss pass @ transportation Get entry level job @ Goodwill Opportunity event - yes to networking Life event - yes to eating out Went to ILAP and seeing this issue, but now it’s only happening on the 2nd opportunity in the list

Get help preparing your immigration app at ILAP Volunteer at Preble Street Workforce development class at Community Financial Literacy Class managing small business at Community Financial Literacy

Job training workshop at Portland Adult Ed - got +1 J immediately Sign up for English @ Hope Acts Life event - yes to purchase bike Went to Hope Acts and clicked “Attend a citizenship class” and clicked submit. The opportunity in the list didn't show that it had been used. I clicked it again and saw this:

Still happening: late in game, Greater Portland Health. Took both "primary care provider" and "Gain MedAccess..." opportunities, then "Discover more locations to build community" --> got this:

Now, there's no phantom box. I said yes to an opportunity, and in the list it appeared as though I didn't take it. So when I clicked it again, I got this box and had no option to click out of it:
