Not able to get my previous entries in midterm survey #110

About: Midterm Survey Application

What steps will reproduce the problem?
1. enter the details..
2. logout and login
3. the previous entries are not coming

What is the expected output? What do you see instead?

I completed half of the survey day before yesterday and opened today to
complete the survey, but I'm not getting the previous details..

What version of the product are you using? On what operating system?

Please provide any additional information below.

I have the same problem.  Completed almost the entire survey, clicked submit, 
had to
go away for a while, wanted to finish it now, but am greeted with a blank 
survey. 
Usually I type into a text editor and copypaste to avoid these kinds of 
problems with
long surveys, but wasn't that lucky this time.

... so while retyping the answers won't kill me, I'd like to know if they were
recorded into the system and can be rescued, because I don't really want to 
retype
everything if I don't have to ;)

Oh, one more thing.  I think I had one required field completely blank, if that 
helps
tracking down the issue.
