Why date doesnt work on jquery pop-up form #231

What steps will reproduce the problem?
plz visit www.hotelbhagyoday.com/contact.htm and click on "answer few 
questions" link. Everything other than date jquery doesnt work. I tried many 
different things.

What is the expected output? What do you see instead?

Which Browser?
chrome/firefox/winxp

Please provide any additional information below.
It works on html page. but doesnt work in pop-up form. It seems a jquery 
conflict. Plz guide me. Thanks in advance.
