Add a box to input custom instructions for diagram generation.

Currently, the Visualize (diagrams generator) is not considering any custom instruction, but trying to come up with valid diagrams in PlantUML/Mermaid languages.

Would be useful to "hint" at which type of diagrams:

Example Instructions:

<< Brainstorm on the implications >>
<< Make it a flowchart >>
<< Huge >>

Requirements:

add an instruction input box, in the style of the other elements (label on top, input box)
if input is empty (or non meaningful, e.g. " ", or "...") ignore
if set, mix it into the prompt where it makes more sense
done!
