Update changelog master after release of 5.0.0 #267

This PR simply updates the changelog to match the updates that were made on maint/5.0. It also adds the release date.

@aaronayres35 when merging this PR, I'd update the commit message - there's a lot of unnecessary info in there about PRs backported to the maintenance branch and then cherry-picked back into master.

I think I will just remove the entire extended description. With squash merge, all we really want is the PR title
