Its cause is still unknown, but I've observed it at least half a dozen times. Will keep looking.

I've experienced it too, but so far I haven't been able to find the cause. I wonder if it's not a Firefox bug? Does this happens with other vertical tabs extensions?

Thanks for confirming. I want to say that editing the add-on's custom CSS might increase the chance of this bug happening.

I'm not sure. I haven't used anything else for an extended period of time.
