refactor Backend DB

[tschimbr]

First port everything including development and test to PostGres.

Attributes

• Rename attribute_types to attributes
• Rename category to type, add categories array
• add values, values_de, values_fr, values_it arrays for categorical attributes (see https://github.com/eonum/casematch/blob/master/app/models/variable.rb)
• add index on code

Hospital attributes

• add index on code
• add year values (Hash)

Remove Uploads
