Protocol guides (and probably other docs) currently use the first heading (#) to encode the main sections of the doc. This is allowed by the markdown specs. However, docusaurus and other markdown renderers reserve the first heading (#) for the page title, which means there should be only one first heading at the top of the file, if any.

fix the headings of the protocol docs by demoting all headings one level down, so that:
1st-level headings (#) become 2nd-level headings (##)
2nd-level headings (##) become 3rd-level headings (###)
