Remove extra classes to prepare for data vectorization #47

replace Vec3d by ndarray as the data type that gets passed around
remove Node, it is not used
merge ParticleToMeshMap into SpatialMesh
replace InnerRegionsManager by a list
replace ExternalFieldsManager by two lists
merge ParticleSourcesManager into Domain
