Adds help and reset form to the top of the page #228

A bit confused by this one, why do we need buttons on the top and bottom?

This is for two reasons:

  1. I am copying what happens on the search results page where the action buttons are at the top and bottom (please see screenshot).
  2. Because this is an accessibility issue I thought it may be easier for someone tabbing through the page who wants to get to the help button to have it in both locations.
