SMDB - Middle name to render on signed data destruction and HIPAA revocation forms #734

If a participant signs the data destruction and/or HIPAA revocation forms (in the PWA) using a middle name, the middle name should also render on the signed PDF forms stored in the SMDB.

@jhflorey Since you're working on both the Data Destruction elements for SMDB and this middle name issue, please address the requirement Aileen states above. thanks!

Update based on the work with forms in PWA - this should include the middle name and suffix (if entered in the signature). Currently the data destruction and HIPAA revocation forms downloaded from the SMDB are displaying the signature variables from the consent form. Please use data destruction and HIPAA revocation concept IDs from issue episphere/connectApp#537

I believe this was fixed in the August release
