Change the application's name

At a conference, @meredithshiels got feedback that the "Epi" prefix might not mean anything to a lot of users.

We should come up with another name. I think it should have the following properties:

• Be concise
• Be easily Googleable (not clash with any other names)
• Be recognizable even if we expand our data from cancer to other diseases.

Wayne suggested "Cancer Tracker", which is a good start, but might be a bit generic when we remove cancer.

Maybe something like Health Disparity Tracker: Cancer? Covers the health disparities angle, and keeps the "tracker" name. But nothing about mortality.

We could also consider other names for "Tracker". Tracker implies a temporal element to me, and our tool isn't yet particularly temporal. Words like "Lens" and "Atlas" and "Scope" and "Radar" are standard. "Monitor" might be good, but that also implies a temporal element to me.

Or perhaps something catchier, like a "Gap" suffix? e.g. CancerGap or HealthGap: Cancer
