This PR addresses:

Responsive sizing for the grids.
Improved accessible targeting for the grids.
Grid misalignment in renderer should be fixed.
Focus on the question text for screen readers (next, back, and modal close buttons.
Handle placeholder for all isDefined min & max numeric values.

@danielruss, could you take a look at this PR when you have some time? This PR updates accessibility and resolves the issues pointed out by IMS in https://github.com/episphere/quest/issues/438.
