@cunnaneaq , The Questionnaire has this question as HORMED, but the data dictionary and mark-up have it as HORMED1. I have updated the Questionnaire. Please review it and if you agree, accept the changes. https://app.box.com/file/1584067199969

@woodruffr I accepted this change in the qx document, I also updated the Spanish mod 2 doc so they match
