Changes in Markdown #25

Recently GitHub made some changes in its Markdown formatting support. As a result, many (hundreds!) links in our specs are now displayed incorrectly. It seems that there now cannot be a link-break between ] and ( characters. This requires minor fixes in all .md files.

All fixed in HEAD versions. Unfortunately, broken formatting will still be visible in all older tags (unless GitHub decides to revert the change).
