Added Frequent issues and fixes #142

There had been many "issues" reported because trivial setup mistakes or other config problems, so I though it would be a good idea to have them documented here.

Hey @erebusnz any possibility of including this? Will reduce the number of irrelevant issues being raised by a lot, IMO. :)
