Please provide age range and tabulation among patients that died within 90 days. May be inappropriate to extrapolate data from 35-year-olds if nearly all cases of mortality occur in 60+ group.

Review: We agree and now present this data in table 2 referenced on line 189-190. We have also changed the recommended age range to 57-95 years (the observed age range for patients who died; line 231). We have also changed the first and last example on line 236-242. Finally, we have imposed a new age limit to the online web calculator.
