Refactored MIDI drastically (static mapping), and half-solved the re-enumeration problem.

The MIDI module now catches portmidi errors, and supports unplugging & re-plugging devices... but they had to be plugged in at the start. This seems to be a problem with PM though?

I'm closing this issue. I'm not going to solve the hot-plugging problem immediately because most other applications don't really support this either. (Maybe much later...)
