[docs] Note URI encoding when refreshing OAuth tokens #59

The docs have been great so far. This is the only thing that stumped me, and may be causing others to struggle when implementing token refresh.

Thanks for the PR!

Would you mind moving the encoding notice(s) a level up? It's a application/x-www-form-urlencoded request, so all parameters are supposed to be URL encoded.
