This draft incomplete PR tracks the incomplete proposed improvement to the cache proposed by Luca in PR #57 at https://github.com/esl/tea/pull/57#issuecomment-31379946.

As discussed with Ed, Luca will propose another iteration of this PR:

Showing why/if the old version of the code is broken;
Clarifying relationship to how deterministic the decision tree is in case of multiple missing dimensions, and to PR #46.
