Add “title” for subject entered choices #14

The "data" field could work as a description field

  1. Add “title” field in UI in /allourideas.org/app/views/items/_form.html.haml
  2. Modify /allourideas.org/app/controllers/questions_controller.rb to include the title field in the choice that is created.
