Add .rnc grammar, & build .rng output from that. #23

If you want to do this, then this PR supersedes #21 and #22 (it incorporates those changes, while making the .rnc grammar the source going forward).

I will do that, even if from the other way round. Producing the rnc from rng. I'll write a new issue about that.
