Initial survey right after consent signature, and prior to going to home screen for first time:
• Age of patient at time of study entry
• Height of patient (one time value at time of study entry)
• Current weight at time of study entry
Two survey questions right after EPV data is entered and they click Done:
• Weight of patient (current with each EPV reading)
• Blood pressure (supplied by doctor's office at time of each EPV clinical visit); two numbers: systolic (larger number), and diastolic (smaller number)
End of study outcome survey questions:
New section with this text: "When your baby is delivered"
• Date of delivery of child (we can derive gestational age from previous GA readings) (wheels or ResearchKit method)
• Birth weight of child in grams (type is data)
• Gender of child: three choices: male, female, other.
• Apgars (two numbers, one for one minute, one for five minutes; virtually all newborns have these values collected)
• Clinical condition of child (one choice from a list, including such options as: healthy, neonatal intensive care unit, premature, intrauterine fetal demise, neonatal death)
Once "Baby Delivered" task is entered, disable enter EPV task, keep graph.
