implement edit profile changes - optional personal name and public certificate #230

This pull request comprises changes to the player 'Edit Profile' page, including two new elements:

  1. An input field where a player can specify an optional personal name.
  2. A new checkbox where a player can specify whether their achievements certificate should be made public.
