Document and improve @ethdebug/pointers integration tests #94

Among other changes, this PR primarily does two things:

Add a section to the "Dereferencing pointers" implementation guide about the integration testing strategy that the reference implementation uses.
Add a second integration test for pointers to uint256[] memory

This PR also refactors a bunch of the code and fixes several bugs that were uncovered in the course of adding the new integration test.
