Minor editorial tweaks - Githubissues

clean up a bogus copy of [R1], by moving the canonical requirement to the second time it is presented (after key terms are all defined).
Suggested convention change: Instead of [**[Rxx]**](#rxx) use **[Rxx](#rxx))** because it seems slightly easier to recognise, search, and edit. I did it in one instance, and it isn't an important change, just one I would find slightly helpful. In other words, I don't care if it gets rejected.
Added myself to contributors. Which is presumptuous of me given it says there that the contributions are gratefully acknowledged, but I hope on balance that's true :)

These changes are purely editorial - they don't effect conformance or implementation.
