Language consistency fixes in README files (CCSM) #81

As in earlier versions of the standard, the initialism “CCSM” is used in the README files with the inconsistent expansion “Distributed Ledger Technology.” This branch replaces the expansion with “Consensus Controlled State Machine”, consistent with the more recent standard and the initials.
