Refactors the add-chain package to more closely align with ethereum-optimism/optimism/op-node file and directory structure.

Refactors add-chain env vars to more closely align with ethereum-optimism/optimism/op-node. (Replaces https://github.com/ethereum-optimism/superchain-registry/pull/317)
