Add QFI questions to PSE Sponsorship Form

Hi there! We will need to add questions from the QFI (Quadratic Funding Initiative) to the current PSE Sponsorship form

• on the second sheet, 'QF'
• Questions should appear once the Category field is chosen

Lastly, can we please change the title from Submit Sponsorship Application to Privacy & Scaling Explorations to ----> Submit Application to Privacy & Scaling Explorations
