[OTHER] Refactor relations between external exposure, exposure events and exposure level #20

I think the current relations are not suitable for representing "typical" external exposures.

The exposure level should be specified at the level of an exposure event and, as mentioned in issue #1, the relation between exposure event and duration is also confusing. I propose an alternative in the line of the diagram below:
